Are you an experienced CHC Nurse Assessor looking for your next interim CHC contract in the North West? Are you passionate about working collaboratively, utilising your clinical expertise in a specialist field? If so, a CHC team in Liverpool is looking for an experienced CHC Nurse Assessor for an interim contract completing assessments to determine eligibility for CHC and FNC funding across Merseyside.

This role would be suitable for an experienced CHC Nurse Assessor with a thorough knowledge of the CHC National Framework who can hit the ground running. The role will involve supporting with all areas of CHC including DST assessments, Reviews, Fast Track and Appeals where required.

Full or part time hours are considered.
The successful candidate can expect attractive pay rates of £250-£280 per day

The responsibilities of the CHC Nurse Assessor will include:
- involvement in all areas of CHC including DST assessments, MDT's and Reviews
- Utilising extensive knowledge of CHC framework
- process urgent fast track referrals and where appropriate commission packages of care
- Providing expert advice to relatives and members of MDT in relation to CHC/FNC funding
- Completing written reports outlining recommendation of eligibility
- Submitting completed written reports in a timely manner

The successful CHC Nurse Assessor will be able to demonstrate:
- Strong understanding of the CHC National Framework
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills

The minimum requirements for this role are:
- NMC registered
- Nursing qualification
