Are you an experienced Social Worker within Adults Social Services, passionate about ensuring the people you work with receive the help they need? If so, Darlington Council are looking for a Social Worker to work in the Older Persons Complex Assessment Team in the Darlington area. The contract is initially 3 months however, may be extended.

You will be working within a supportive team with a committed leadership team which is focused on improving the outcomes of service users across Darlington.

As a Social Worker for the Older Persons Complex Assessment Team, you will carry a caseload of assessments for service users primarily aged 65+ ensuring the correct care packages are in place for service users.

This is a full time role working 5 days per week.

The successful candidate can expect attractive pay rates of up to £187.77 per day.

As a Social Worker you will be responsible for:

To identify, in conjunction with individuals and or their carers, the most appropriate ways of achieving outcomes identified through the assessment process and to promote independence and well-being through the use of universal, targeted and specialist services as appropriate.
Ensure they have access to appropriate representation including advocacy, IMCA, IMHA where appropriate..
Provide advice and casework support to other Social Workers in relation to their cases. Develop relationships with colleagues in own and other departments and external agencies in order to gather information relevant to assessment and support planning activities and ensure effective partnership working takes place promoting positive outcomes for individuals.
Maintain and update case notes and other records, write reports as required; if required give evidence in court.

The minimum requirements for this Social Worker role are:

-HCPC Qualification

-Social Work Qualification

-Experienced at a Senior Manager level within Children's Services
