Are you an experienced RNLD CHC Nurse Assessor, eager to start your next contract as part of a well established and supportive CHC team in the West Midlands? Are you looking to work with an agency that will support you with your professional development? If so, a CHC team in Birmingham is looking for Learning Disabilities Nurse (RNLD) with CHC experience for a 3 to 6 month contract.

This role would be suitable for someone who is able to demonstrate excellent knowledge and experience determining eligibility of CHC/FNC funding following the CHC national framework as well as have a background working with patients with complex learning disabilities, transitioning from children's to adults services and is comfortable with complex case management duties.

Full time (4 days negotiable)
3 to 6 month contract
£300 - £310 per day (Inside IR35)

The responsibilities of the CHC Nurse Assessor will include:
- Determine the eligibility of NHS CHC and FNC funding utilising the CHC National Framework
- Working closely with families and patients to ensure the timely completion of assessments
- Making appropriate recommendations to CCG's and CSU's, and writing up cases to be presented at panel meetings.
- Joint working, developing strong links health and social care colleagues, to ensure effective working.
- Liaising with local authority teams to discuss any joint-funded applications/reviews
- Arranging and taking part in Continuing Healthcare assessments with other members of an MDT.

The successful candidate will be able to demonstrate:
- Strong understanding of the CHC National Framework
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills with the confidence to work with individuals with varying clinical conditions

The minimum requirements for this role are:
- NMC registered (RNLD Preferred)
- Nursing qualification
