Are you a diligent CHC Nurse Assessor, who is passionate about ensuring your clients receive the right levels of support? Do you seek a work environment within which you know you will be valued and your hard work will be appreciated? If the answer is 'yes', a CHC team in Leicester is looking for an experienced CHC Nurse Assessor to support with a project of CHC Reviews across Leicester, Rutland and Leicestershire starting in January 2019.

The role is suitable for a healthcare professional with a clear understanding of how to complete CHC and FNC reviews, as well as the funding eligibility process.

Full time hours (37.5) preferred however 4 days per week will also be considered. Flexible working, including home working if also considered.
Daily rate of £250-280 per day (Inside IR35)
This is a 3 month contract initially with potential of further extension

The responsibilities of the CHC Nurse Assessor will include:
- Completing 3 monthly and 12 monthly reviews of clients in receipt of FNC or CHC funding.
- Utilising extensive knowledge of CHC framework
- Providing advice to relatives and members of MDT in relation to CHC/FNC funding
- Completing written reports outlining recommendation of eligibility, and reviewing care packages
- Submitting completed written reports in a timely manner

The successful CHC Nurse Assessor will be able to demonstrate:
- Strong understanding of the CHC National Framework
- The ability to complete strong, evidence-based FNC/CHC reviews
- An ability to work autonomously and self-motivate
- Excellent written and verbal communication
- Excellent people management skills with the confidence to work with individuals with varying clinical conditions

The minimum requirements for this role are:
- NMC registered
- Nursing qualification
