Are you an experienced CHC Nurse Assessor looking for your next interim CHC contract? Are you passionate about working collaboratively with others by sharing your expert knowledge of continuing healthcare? If so, a CHC team in Preston, Lancashire is looking for an experienced CHC Nurse Assessor for an interim contract completing assessments to determine eligibility for CHC and FNC funding. These opportunities are available across North and East Lancashire.

This role is suitable for CHC Nurse Assessor with a wealth of knowledge and experience of the National Framework.

Full or part time hours are considered.
The successful candidate can expect attractive pay rates of £250-£300 per day

The responsibilities of the CHC Nurse Assessor will include:
- involvement in all areas of CHC including DST assessments, MDT's and Reviews
- Utilising extensive knowledge of CHC framework
- Procurement of appropriate packages of care for individual's eligible for NHS Continuing Care and recording of eligibility for FNC payment in order for the weekly care contribution to be paid
- process urgent fast track referrals and where appropriate commission packages of care
- Providing expert advice to relatives and members of MDT in relation to CHC/FNC funding
- Completing written reports outlining recommendation of eligibility
- Submitting completed written reports in a timely manner

The successful CHC Nurse Assessor will be able to demonstrate:
- Thorough understanding of the CHC National Framework
- Knowledge of Personal Health Budgets (PHBs) is desirable
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills

The minimum requirements for this role are:
- NMC registered
- Nursing qualification
