If you are passionate community psychiatric nurse looking for a new exciting opportunity to develop your career and feel your experience in mental health services would help care for those who are suffering from mental health problems, this could be a great role for you. We are looking for a community psychiatric nurse to join a community mental health team were you would be required to implement vital treatment and care to working age adults (18-65) who are experiencing difficulties in their mental health. Working as a community psychiatric nurse within the team you will be required to work to identify safe and progressive care in the hope of preventing the need for hospital admissions.

Day to day tasks in this role would include: supporting services, providing risk assessments, creating care plans and making referrals. The role is based in Preston , close to public transport, shops and restaurants. If you feel your attributes would be suitable for this role please read more information below. The ideal candidate would be a band 5 or 6 with experience in working with in a community mental health setting.

This role is to cover a vacancy within the team and is expected to run for at least 3-4 months, with possibility of extension. This role is to work between 30-37.5hrs per week over 5 days (Monday to Friday). There is the option to work shifts on a more flexible basis or work part time.

As a community psychiatric nurse working within a community mental health team your duties would include:

* Making home visits or community
* Making visits to hospital if someone is going on leave or being discharged
* Assessing the needs of the individual and deciding the next steps, offering support to help patients stay at home
* Assist with self-help strategies
* Administering medication, in a home or clinical setting
* Provide practical help, for example with money, housing or childcare arrangements
* Helping to plan future care to someone to help them manage their mental health
* Carrying out urgent mental health assessments to those in a state of crisis
* Discharging those who no longer require the service when appropriate
* Working closely with local authority when needed
* Making referrals over to other services when necessary

Minim requirements for this role are:

- A degree in Mental Health Nursing
- Valid NMC Pin
- Experience working within community mental health
