Are you an experienced Registered Nurse with extensive knowledge of CHC looking to work within a supportive Continuing Healthcare team? If so, a CHC team in Nottingham require a CHC Nurse Assessor for a 3 month contract to support with DST assessments in Nottinghamshire.

As a CHC Nurse Assessor, you will be responsible for chairing MDT meetings, working with other health and social care professionals completing DST assessments to determine patient eligibility for CHC funding.

This is a full time post however can be flexible to work 4 days per week.
Competitive pay rates on offer of £280-300 per day dependent on experience.

The responsibilities of the CHC Nurse Assessor will include:
- Completing DST Assessment to determine eligibility of CHC/FNC funding, assessing change in their current health needs.
- Utilising extensive knowledge of CHC framework
- Providing advice to relatives and members of MDT in relation to CHC/FNC funding
- Completing written reports outlining recommendation of eligibility
- Submitting completed written reports in a timely manner

The successful candidate will be able to demonstrate:
- Strong understanding of the CHC National Framework
- Highly motivated and able to work autonomously
- Excellent written and verbal communication
- Excellent people management skills

The minimum requirements for this role are:
- NMC registered
- Nursing qualification (RGN, RMN, RNLD, RSCN)
