Are you looking to continue the fulfilling work you have previously done working as a cpn or rmn?You will conduct initial assessments, agree formulation of need and develop collaborative care plans underpinned by NICE Guidance delivering a range of interventions based upon individual needs. Assessments and interventions can take place in patient's homes, team bases, hospital and other community settings or via tele-coaching and through the use of assisted technology and on line forums.
