Health Care Support Worker - Swansea (28 hours)

Do you have a passion for providing first class care? Are you an experienced Health Care worker? We have exciting opportunities available in the Swansea area. You will be working as part of the Home from Hospital Service.

You will provide personal care, practical and emotional support to vulnerable persons within Swansea and/or Neath Port Talbot who are identified as requiring support in order to remain in their own home and avoid the potential for unnecessary hospital admission.

Working hours are 4 X 7 hour shifts to be agreed Mon-Fri (Shifts will be between 9am & 5pm), however, evening and weekend cover on a rota basis may be required in line with future service development.

Responsibilities:



* With support from the Team Leader as necessary to undertake a full and comprehensive assessment of need


* To undertake necessary risk assessments in accordance with the needs and as identified by the referrer.


* To deliver care directly undertaking and supervising tasks related to the physical (including personal care), social and emotional needs of service users.
* To communicate with patients, carers and health professionals in a professional manner
* To assist the patient with oral medication when the patient is self - medicating
* To record any changes in the patients circumstances, condition or symptoms and notify other health care professionals as necessary
* To notify the Team leader, Service Manager and Independent living Manager of any accidents, incidents and near misses and to report and record them in line with BRC best practice and policies and procedures
* To be aware of, identify and report any safeguarding concerns to the Team leader, Service Manager and or Independent living Operations Manager and ensure all reporting procedures are followed in accordance with policies and procedures.
* To ensure that welfare checks, as required, are conducted and to liaise where necessary with the service users relatives and neighbours
* To update patient records following each and every visit and ensure the hand - over of care is in accordance with agreed policies and procedure
* To preserve the dignity and confidentiality of the service user at all times
* To liaise with and provide support to the service users family and carers throughout the episode of care and support
* To ensure that welfare checks, as required, are conducted and to liaise where necessary with the service users relatives and neighbours
* To update patient records following each and every visit and ensure the hand - over of care is in accordance with agreed policies and procedures
* To preserve the dignity and confidentiality of the service user at all times
* To ensure that volunteers working in the service complete the required paperwork to support service delivery
* To report to the Team Leader after each duty and ensure that the Lone Worker policy is fully complied with.
* To apply learning from training, complaints, client engagement activities and other professional development experiences
* To work within the policies and procedures of the organisation.

You will have at least 6 months experience of working within a residential setting. Other requirements include:



* A valid DBS
* Driver with access to own vehicle
* NVQ level 3 in health and social care or willingness to undertake QCF level 3
* Good verbal and written communication skills
* Understanding of issues relating to the care and support of vulnerable people preferably in a domiciliary care setting
* Has empathy and understanding of vulnerable adults
* 2 years experience of working within community based health and social care Statutory or voluntary sector service
* Ability to work alone without direct supervision and as part of a team
* Understanding of and the ability to maintain confidentiality
* Able to work and travel throughout the area served by the scheme as appropriate to the needs of the job
* Full driving licence and access to a vehicle
