To deliver person-centred, strengths based support to people accessing Supported Housing services, and line manage other Support workers in the supported housing service. Direct line management of Coaches within the service, ensuring that the key accountability of their role are delivered effectively and consistently.Manage a a team of caseload coaches and a caseload of service users through their experience with the supported housing service, enabling them to articulate their aspirations and realise their potential. Manage referrals into the service, undertake interviews and assessments, and notify potential service users of the outcomes. Induct new service users, including the provision of information regarding Rights and Responsibilities, and expectations regarding the customer's commitment to engage in support sessions.Deliver quality, outcome focused support sessions in both one-to-one sessions and group sessions; including but not limited to support sessions on finance and budget management, healthy relationships, mental health and well-being and employment skills, ensuring customers complete relevant paperwork..Maintain up to date risk assessments and ensure compliance with Safeguarding policies and procedures; appropriately managing safeguarding concerns to ensure excellent safeguarding practice is embedded within your work.Identify and signpost any support they may benefit from that is available from partner agencies, supporting service users to access these services as appropriate.Support individuals to pay their rent, service charge and any personal charges in accordance with their license agreement. Encourage the views on the service provided and explore any ideas / improvements. Carry out health and safety checks and risk assessments of the building as requested by your line manager, update associated records and address any identified risks, repairs, etc.
The CandidateProven experience of working with vulnerable people with support needs. Ideally with team Management experience.Proven experience of caseload management, with a clear focus on outcomes for customers and their wider community.Ability to demonstrate partnership working with specialist agencies such as mental health teams, offending services or other relevant services.Working knowledge of housing and benefits advice.Proven experience of line management within a supported housing environment, or similar working environment.Excellent communication skills, with the ability to work in a professional manner with a number of different agencies, including representation at multi-disciplinary meetings.Ability to work under pressure and using your own initiative.Ability to work positively and creatively with support needs.Numerate and literate to GCSE standard, IT literate and able to use all Microsoft packages.Level 3 qualification i.e. Health and Social Care (or equivalent).DBS - on Update Service or within last 2 months
Please Note -- Due to the nature of the role you will be flexible in your approach, working hours on a seven day rota which may include bank holidays, evenings, weekends and days on a rota basis.
