- Level 3 social worker to join busy hospital social work team
- The main area of the work is to arrange safe and timely discharges for both patients admitted onto acute, non acute wards and those people attending the Accident and Emergency Department.
- You will need to have at least 2 years relevant experience and hold a social work degree/or social work qualification and have up to date HCPC registration.
- You will manage a complex caseload undertaking social care assessments and mental capacity assessments to ensure safe and timely discharges.
- You will deliver exceptional social care to adult service users and their carers which enables them to make their own decision.
-You will need to operate within an agreed performance management and budgetary framework. A significant part of your role will be to identify risks to the individual and respond appropriately thus ensuring any safeguarding issues are addressed.

- Degree in Social Work or equivalent
- Member of the Health Care Professions Council (HCPC)
- Excellent organisational skills and the ability to work flexibly, under direction and independently where necessary
