The successful candidate would receive referrals from wards in the hospital and from the A and E department and would carry out assessments. They would develop a care plan based on the service users needs and where appropriate, refer patients onto other services to ensure they are receiving suitable treatment.The shifts are ad hoc and can be over days, nights, weekends or weekdays, chosen at your convenience. Shifts can be ad hoc with the potential to be block booked on a longer term basis is desired.
