Job Overview:

We are seeking a skilled and compassionate Social Worker to join the Disabilities Team in Sutton. The successful candidate will work with adults and/or children with physical, learning, or developmental disabilities, supporting their independence and well-being through assessments, care planning, and multi-agency collaboration.

Key Responsibilities:

• Carry out strengths-based assessments under the Care Act 2014 and Children and Families Act 2014 (as appropriate).

• Develop and implement support plans that promote independence, inclusion, and well-being.

• Conduct risk assessments and manage safeguarding concerns in line with statutory guidelines.

• Work closely with health professionals, education providers, families, and other stakeholders.

• Ensure that services provided meet quality standards and are person-centered.

• Maintain accurate, up-to-date case records and documentation.

• Participate in regular supervision and team meetings, contributing to service improvement.

Requirements:

• Qualified Social Worker with Social Work England registration.

• Experience working with individuals with disabilities, either in adult or children's services.

• Strong working knowledge of the Care Act, Mental Capacity Act, and relevant disability legislation.

• Excellent communication, assessment, and report-writing skills.

• Ability to manage a diverse caseload and work independently as well as part of a team.
