Are you a skilled and compassionate social worker with experience in adult services? We are seeking a dedicated professional to join a Community Review Team, focusing on supporting and enhancing the well-being of adults in our community. This role offers the opportunity to make a positive impact by conducting assessments and coordinating interventions to meet the unique needs of service users.
Key Responsibilities:
- Assessment and Review: Conduct thorough assessments and reviews of adult's social care needs, working collaboratively with individuals, families, and relevant stakeholders.
- Person-Centred Planning: Develop and implement person-centred care plans, ensuring that interventions are tailored to the unique needs and preferences of each service user.
- Community Engagement: Work closely with community resources, healthcare professionals, and other support services to enhance the overall well-being of individuals within the community.
- Advocacy: Advocate for the rights and choices of service users, ensuring their voices are heard and respected in decision-making processes.
- Risk Assessment: Conduct risk assessments and develop risk management plans in collaboration with the service user and other professionals as needed.
- Documentation: Maintain accurate and up-to-date records of assessments, care plans, and interventions in accordance with professional and organisational standards.
Qualifications and Experience:
- Qualified Social Worker with relevant professional qualifications.
- SSSC Registration
- Previous experience in reviewing and especially around SDS and Direct Payment funding.
- Skills and Abilities:
- Strong interpersonal and communication skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Empathy and cultural competence in working with diverse populations.
- Knowledge of relevant legislation and policies in adult social care.
