You will be responsible for managing a caseload and completing assessments , support plans and reviews with audits. Completing Court of Protection documents, best interests and safeguarding investigations. As part of the role to assess risk, explore suitable placements (Supported Living, residential and housing) alongside health professionals.
The ideal candidate will be experienced in the sector and will be confident in their role. You must be IT literate and prefer to have Liquid Logic experience, however training will be provided if you are not familiar with Liquid Logic.
Responsibilities:
- Conduct comprehensive assessments of adults' social care needs, including physical, mental health, housing, and social support requirements.
- Develop and implement person-centred care plans in collaboration with clients, families, and relevant professionals.
- Review and update care plans regularly to reflect changes in clients' circumstances and goals.
- Provide practical and emotional support to adults to enable them to maintain independence, dignity, and quality of life.
- Deliver evidence-based interventions and therapeutic support to address issues such as loneliness, isolation, mental health challenges, and substance misuse.
- Advocate for clients' rights and access to appropriate services, including healthcare, housing, benefits, and community resources.
- Identify and assess risks to adults' safety and well-being, including concerns related to abuse, neglect, exploitation, or self-neglect.
- Take appropriate safeguarding measures, including reporting concerns to safeguarding authorities and coordinating safeguarding plans with relevant agencies.
- Monitor and review safeguarding arrangements to ensure ongoing protection and support for vulnerable adults.
- Coordinate the delivery of services and support from internal and external providers, including social care teams, health services, housing agencies, and voluntary organisations.
- Collaborate with multidisciplinary teams, including health professionals, psychologists, occupational therapists, and care managers, to ensure holistic and integrated care for clients.
- Facilitate multi-agency meetings and care reviews to promote communication, collaboration, and person-centred planning.
- Maintain accurate and up-to-date records of assessments, care plans, interventions, and client interactions in accordance with legal and regulatory requirements.
- Prepare reports, case summaries, and other documentation for case conferences, court proceedings, and reviews as necessary.
Qualifications:
- Bachelor's or Master's degree in Social Work or a related field.
- Registration as a Social Worker with the relevant regulatory body.
