Lincolnshire council are recruiting Qualified Adult Social Workers. The rate of pay is up to £35 per hour dependant on experience.
Responsibilities:
Assessment and Care Planning:
- Conduct thorough assessments of individuals' physical, emotional, and social needs.
- Develop and implement personalized care plans in collaboration with clients, their families, and relevant professionals.
- Regularly review and update care plans to reflect changing needs and circumstances.
Support and Advocacy:
- Provide emotional support, advice, and guidance to individuals and their families.
- Advocate for the rights and interests of vulnerable adults, ensuring their voices are heard and respected.
- Connect clients with appropriate community resources and services to enhance their well-being.
Risk Management and Safeguarding:
- Identify and evaluate potential risks and safeguarding concerns, taking appropriate actions to protect individuals from harm.
- Collaborate with colleagues and agencies to implement safeguarding measures and interventions as necessary.
Liaison and Collaboration:
- Work closely with multidisciplinary teams, including healthcare professionals, housing agencies, and mental health services, to coordinate holistic care.
- Maintain effective communication with colleagues, supervisors, and external partners to ensure seamless service delivery.
Documentation and Record Keeping:
- Maintain accurate and up-to-date case records, documenting assessments, interventions, and outcomes.
- Ensure that all documentation adheres to legal and regulatory standards.
Continuous Professional Development:
- Stay informed about developments in social work practice, policies, and legislation.
- Engage in regular training and professional development activities to enhance your skills and knowledge.
Requirements:
- A recognized qualification in social work (e.g., Bachelor's or Master's degree in Social Work).
- Registered with the Health and Care Professions Council (HCPC) as a qualified social worker.
- Previous experience in adult social work or a related field is preferred.
- Strong assessment, communication, and interpersonal skills.
- Knowledge of relevant legislation, policies, and procedures related to adult social care.
- Ability to work independently and as part of a team.
- Empathy, patience, and a non-judgmental approach to working with diverse individuals and communities.
