Job Title: Social Worker 14+ Team
Location: Vale of Glamorgan
Salary: £40-£42ph
As a Social Worker in the 14+ Team, you will play a vital role in supporting young people aged 14 and above as they navigate the challenges of adolescence and prepare for adulthood. You will work closely with young people, their families, and other professionals to assess their needs, develop care plans, and coordinate appropriate support services to help them achieve their goals and aspirations.
Job Description:
Key Responsibilities:
- Provide direct support and advocacy to young people aged 14 and above, helping them to access education, training, employment, housing, and other essential services.
- Conduct assessments of young people's needs and risks, taking into account their strengths, preferences, and aspirations.
- Develop and implement care plans in collaboration with young people, their families, and other professionals, ensuring that services are tailored to meet their individual needs and goals.
- Support young people to develop independent living skills, including budgeting, cooking, and accessing community resources.
- Work collaboratively with colleagues, partner agencies, and other professionals to ensure a coordinated and holistic approach to supporting young people's needs.
- Maintain accurate and up-to-date records of all interactions, assessments, and interventions, ensuring compliance with legal and regulatory requirements.
Requirements:
- Degree in Social Work or equivalent qualification recognized by Social Care Wales.
- Registered with Social Care Wales as a Social Worker.
- Previous experience working with young people in a social work or related setting.
- Strong assessment and communication skills, with the ability to engage effectively with young people from diverse backgrounds.
- Knowledge of relevant legislation, policies, and procedures related to children and young people's services.
- Commitment to promoting the rights and well-being of young people and empowering them to achieve their full potential.
