Job Title: Social Worker Children with Disabilities
Location: Vale of Glamorgan
Salary: £40-£42ph
As a Social Worker in the Children with Disabilities Team, you will be responsible for providing direct support to children with disabilities and their families, assessing their needs, and coordinating appropriate services and interventions. You will work collaboratively with families, partner agencies, and other professionals to ensure that children with disabilities receive the care and support they require to achieve their full potential and participate fully in community life
Key Responsibilities:
- Conduct assessments of the needs and strengths of children with disabilities and their families, taking into account their individual circumstances, preferences, and aspirations.
- Develop and implement care plans in collaboration with children, families, and other professionals, ensuring that services are tailored to meet their specific needs and goals.
- Provide direct support and advocacy to children with disabilities and their families, helping them to access a range of services and resources, including education, health-care, and social support.
- Work closely with partner agencies and other professionals to coordinate and monitor the delivery of services and interventions, ensuring that they are effective and appropriate.
- Maintain accurate and up-to-date records of all interactions, assessments, and interventions, ensuring compliance with legal and regulatory requirements.
- Participate in team meetings, case conferences, and training sessions to share knowledge and expertise and contribute to the continuous improvement of service delivery.
Requirements:
- Degree in Social Work or equivalent qualification recognised by Social Care Wales.
- Registered with Social Care Wales as a Social Worker.
- Previous experience working with children with disabilities and their families in a social work or related setting.
- Strong assessment and communication skills, with the ability to engage effectively with children and families from diverse backgrounds.
- Knowledge of relevant legislation, policies, and procedures related to children with disabilities and their families.
- Commitment to promoting the rights and well-being of children with disabilities and empowering them to achieve their full potential.
