As part of the Adults Locality Team in Gateshead, you will provide vital support and care to adults with a range of health and social care needs within the community. Your role will involve working closely with individuals, families, and other professionals to promote independence, wellbeing, and inclusion.
Key Responsibilities:
Deliver person-centred care and support to adults within the community, promoting independence and choice.
Carry out assessments and contribute to care planning in collaboration with service users and other professionals.
Support individuals with daily living activities, including personal care, medication management, and accessing community resources.
Monitor and report changes in service users' health, wellbeing, or social circumstances.
Work closely with multidisciplinary teams, including social workers, health professionals, and voluntary agencies.
Ensure compliance with safeguarding procedures and promote the safety and dignity of all service users.
Maintain accurate and timely records of support activities, incidents, and outcomes.
Participate in team meetings, training, and supervision to develop skills and knowledge.
Person Specification:
Experience working with adults in a health or social care setting.
Understanding of social care principles and person-centred practice.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Commitment to safeguarding and promoting the welfare of vulnerable adults.
Flexibility to work across different shifts and locations within the locality as required.
Relevant qualifications or willingness to work towards qualifications in social care (e.g., CACHE, NVQ, or equivalent) are desirable.
