Stockton-on-Tees Borough Council's Children with Disabilities (CWD) team is seeking a dedicated and compassionate Social Worker to join our team. We are committed to providing high-quality support and advocacy for children with disabilities and their families, empowering them to achieve their full potential.
About the Role and Responsibilities:
... - Conducting Comprehensive Assessments: Carry out thorough assessments of children with disabilities and their families, identifying their needs, strengths, and challenges.
- Developing and Implementing Care Plans: Develop and implement individualised care plans, ensuring the safety and well-being of children. This will involve collaborating with families, other professionals, and agencies to create effective support strategies.
- Direct Work with Children and Families: Provide direct support to children and families through one-to-one sessions, group work, and community outreach.
- Facilitating Access to Services: Help children and families access specialist services and resources, such as education, healthcare, and social inclusion opportunities.
- Collaborative Working: Work effectively with other professionals and agencies (health, education, etc.) to ensure children receive holistic and coordinated care.
- Advocacy and Support: Advocate for the needs of children with disabilities, ensuring their rights and well-being are protected.
- Maintaining Accurate Records: Maintain accurate and up-to-date records and documentation, adhering to relevant legislation and guidelines.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Experience working with children with disabilities and their families.
- Knowledge of relevant legislation and guidance related to supporting children with disabilities.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to build rapport with children, families, and other professionals.
- Commitment to supporting children with disabilities to achieve their full potential and live fulfilling lives.
