About the Role:
- Conduct thorough needs assessments and develop person-centered care plans for children with disabilities.
- Provide direct support and guidance to children, young people, and their families, promoting independence and well-being.
- Collaborate with multi-agency partners (health professionals, educational providers, support organizations) to coordinate services and resources.
- Advocate for the rights and needs of children with disabilities, ensuring access to appropriate services and support.
- Manage a caseload effectively, prioritizing urgent needs and adhering to statutory timescales.
- Contribute to a supportive and collaborative team environment focused on continuous improvement.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience working with children with disabilities is highly desirable.
- Excellent assessment, communication, and case management skills.
- Strong understanding of relevant legislation and policies relating to children with disabilities.
- Ability to build rapport and trust with children, young people, and their families.
- Commitment to person-centered practice and ethical standards.
