The Role:
As a Complex Care Nurse Case Manager, you will be pivotal in designing and implementing person-centered care plans, coordinating services from multiple disciplines (GPs, therapists, social workers), overseeing care packages, and driving continuous improvement through data analysis. You'll also play a role in local business development and commissioning.
Key Responsibilities:
- Conduct comprehensive patient assessments
- Develop and implement individualized care plans
- Coordinate care across healthcare disciplines
- Oversee care packages, ensuring safety and effectiveness
- Advocate for patients rights and access to resources
- Educate patients and families
- Monitor patient progress and adapt care plans as needed
- Collaborate with interdisciplinary teams
- Analyze data to improve patient outcomes
- Maintain accurate patient records
What We Offer:
- Highly Competitive Salary: £50,000 - £55,000 per annum (Full Time, DOE)

To Qualify:
- NMC PIN - Registered Nurse in Learning Disabilities or Children's Nursing
- Proven experience in complex care
