Stockton-on-Tees Borough Council is seeking a dedicated and compassionate Qualified Social Worker to join our dynamic First Contact Team. This is a crucial role as the "front door" of Adult Social Care, where you will be the first point of contact for new referrals and inquiries, ensuring vulnerable adults receive the right support at the right time.
Pay Rate & Key Requirements
Pay Rate: £20 - £26 per hour
Essential:
You must be a qualified Social Worker.
Social Care England registration is a must.
An up-to-date DBS check is required.
Roles & Responsibilities
Manage incoming referrals and enquiries, acting as the first point of contact for Adult Social Care.
Conduct initial assessments and screenings to determine the urgency and nature of a person's needs.
Apply the Care Act 2014 and Mental Capacity Act 2005 to all work, ensuring legal compliance and prioritizing the well-being and safety of individuals.
Collaborate with multi-agency partners, including the police, health services, and housing departments, to provide a holistic and coordinated response.
Provide advice and guidance to individuals, their families, and other professionals, using a strength-based approach to promote independence and well-being.
Maintain accurate and detailed records on care management systems, ensuring all information is up-to-date and reflects the work undertaken.
Participate in forums and meetings such as Multi-Agency Public Protection Arrangements (MAPPA) and Multi-Agency Risk Assessment Conferences (MARAC).
