This is an exciting opportunity to make a real difference in the lives of adults within the Gateshead community. As part of our dynamic Adult Locality Team, you will play a crucial role in promoting independence, well-being, and positive outcomes for individuals.
Key Responsibilities:
- Undertake comprehensive Care Act assessments to identify needs and develop person-centred support plans.
- Prepare and manage Court of Protection applications, ensuring the best interests of individuals are upheld.
- Lead on safeguarding enquiries, working collaboratively to protect vulnerable adults from abuse and neglect.
- Participate in the team's duty rota, responding to urgent referrals and providing timely intervention.
- Conduct regular reviews and monitoring of care and support plans, ensuring they remain effective and responsive to changing needs.
- Work collaboratively with individuals, families, carers, and a range of multi-agency partners to achieve positive outcomes.
- Maintain accurate and up-to-date records in line with professional standards and Gateshead Council policies.
About You
- Qualified Social Worker with current Social Work England registration.
- Demonstrable experience of working with adults in a social care setting.
- Thorough knowledge and understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.
- Proven ability to undertake complex assessments and develop effective care plans.
- Experience in undertaking safeguarding enquiries and working with risk.
- Familiarity with Court of Protection processes.
- Excellent communication, interpersonal, and negotiation skills.
- Strong organisational skills and the ability to manage a diverse caseload.
- A commitment to promoting independence, choice, and control for individuals.
- Ability to work effectively both independently and as part of a team.
