Are you a proactive Social Worker or Nurse with a passion for supporting patients in their transition from hospital to home? We are looking for dedicated individuals to join the Newcastle Integrated Hospital Discharge Team to help ensure safe, timely, and effective discharges across the city's major healthcare hubs.
The Role
As part of a fast-paced multidisciplinary team, you will act as the vital link between acute hospital wards and community services. Your goal is to reduce "delayed transfers of care" and ensure patients have the right support packages in place to recover safely.
Key Responsibilities:
Assessment: Conducting holistic assessments of patients' needs under the Care Act (2014) or Continuing Healthcare (CHC) frameworks.
Coordination: Liaising with doctors, therapists, families, and home-care providers to build robust discharge plans.
Problem Solving: Identifying barriers to discharge and implementing creative solutions to support "Home First" initiatives.
Mental Capacity: Completing Mental Capacity Assessments and Best Interest Decisions where required.
What We're Looking For
Qualifications: Must be a Qualified Social Worker (registered with Social Work England) or a Registered Nurse (NMC).
Experience: Previous experience in a hospital setting, rapid response, or intermediate care is highly desirable.
Skills: Strong communication skills and the ability to work under pressure in a high-intensity environment.
Local Knowledge: Familiarity with Newcastle-based community resources and adult social care pathways is a plus.
