Job Title: Adult Hospital Social Worker
Hourly Rate: £20-£26 (dependent on experience)

You will play a vital role in providing high-quality social work services to adult patients, working collaboratively with a multidisciplinary team to assess needs, develop care plans, and support patients through their hospital journey and beyond.

Key Responsibilities:
* Conduct comprehensive assessments of adult patients.
* Develop and implement person-centred care plans, considering physical, emotional, and social needs.
* Collaborate effectively with medical staff, nurses, occupational therapists, and other professionals.
* Advocate for patient's rights and ensure their voices are heard.
* Liaise with families, carers, and other relevant agencies.
* Facilitate timely and appropriate hospital discharges.
* Support patients with their transition back to the community, linking them with necessary services.
* Contribute to the ongoing development and improvement of the team's services.
* Comply with all relevant legislation and professional guidelines.
* Maintain accurate and up-to-date case records.
Essential Criteria:
* A recognised Social Work qualification
* Current registration with the Health and Care Professions Council
* Significant post-qualifying experience in adult social work.
* Experience working within a hospital setting is highly desirable.
* Excellent communication, assessment, and care planning skills.
* Ability to work independently and as part of a multi-disciplinary team.
Desirable Criteria:
* Experience working with specific patient populations (e.g., older adults, people with mental health conditions).
* Knowledge of relevant legislation and policy frameworks.
* Experience with case management software.
