Key Responsibilities:
* Conduct comprehensive assessments of adults with learning disabilities, considering their individual needs, strengths, and aspirations. This will involve using a range of assessment tools and working collaboratively with the individual, their family, and other professionals.
* Develop and implement person-centred care plans that promote independence, choice, and inclusion. This will involve working with the individual to identify their goals and aspirations, and developing a plan to support them to achieve these goals.
* Work collaboratively with individuals, families, carers, and other professionals (e.g., health-care providers, educational staff, advocacy services). This includes regular communication and joint working to ensure that the individual receives a holistic and coordinated approach to their care.
* Advocate for the rights and needs of service users, ensuring their voices are heard and their wishes are respected. This will involve challenging systems and advocating for improvements where necessary.
* Facilitate access to appropriate services and resources, including community-based support. This will involve liaising with other agencies and services to ensure that individuals receive the support they need.
* Support individuals in their own homes and communities. This may involve direct support, practical assistance, and advocacy.
* Maintain accurate and up-to-date case records, adhering to all relevant legislation and professional guidelines. This will involve meticulous record keeping, ensuring confidentiality and compliance with data protection regulations.
* Contribute to the ongoing development and improvement of the team's services. This includes participating in reflective supervision and contributing to team meetings and training.
* Participate in regular supervision and training opportunities. This will provide ongoing professional development and support.
Essential Criteria:
* A recognised Social Work qualification
* Current registration with the Health and Care Professions Council.
* Significant post-qualifying experience in adult social work, with demonstrable experience working with people with learning disabilities.
* Excellent communication, assessment, and care planning skills.
* Ability to work independently and as part of a multi-disciplinary team.
* Full UK driving licence and access to a vehicle.
Desirable Criteria:
* Experience working with specific sub-groups of adults with learning disabilities (e.g., those with challenging behaviours, complex health needs).
* Knowledge of relevant legislation and policy frameworks (e.g., Care Act 2014, Mental Capacity Act 2005).
* Experience with case management software.
