Gateshead Council's Learning Disabilities Team is looking for a dedicated and experienced Social Worker to support adults with learning disabilities. The role focuses on person-centred assessments, care planning, and safeguarding to help individuals live as independently and safely as possible within the community.
Key Responsibilities:
Carry out Care Act 2014 assessments and reviews for individuals with learning disabilities.
Develop outcome-focused, person-centred support plans in collaboration with service users and their carers.
Work within a multi-disciplinary framework, liaising with health professionals, care providers, families, and other stakeholders.
Respond to safeguarding concerns and manage risks appropriately, in accordance with local policies and statutory guidance.
Undertake Mental Capacity Act assessments and contribute to Best Interest decisions where necessary.
Support transitions, hospital discharges, and reviews of long-term placements.
Maintain accurate and timely case records using Gateshead's electronic case management system.
Participate in team meetings, reflective supervision, and service development activities.
Person Specification:
Qualified Social Worker with Social Work England (SWE) registration.
Previous experience working in a community-based learning disabilities team or adult social care setting.
Knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.
Strong assessment, communication, and interpersonal skills.
Ability to manage a varied caseload with competing priorities.
Experience using electronic case recording systems is desirable.
Enhanced DBS clearance is required.
