About the Role
As a Review Social Worker, you will play a crucial role in safeguarding the wellbeing and rights of older adults. Your primary focus will be on the systematic, timely, and holistic review of existing care arrangements.
Key Responsibilities Include:
- Conducting statutory reviews for older adults in accordance with relevant legislation and local policies.
- Working in partnership with service users, carers, families, and multidisciplinary teams (MDTs) to assess ongoing needs and outcomes.
- Developing and updating care and support plans to reflect current needs, promoting independence, choice, and control.
- Identifying and responding to safeguarding concerns promptly and effectively.
- Maintaining accurate and timely records using HSCP systems.
Qualifications and Skills:
- A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC).
- Current SSSC registration as a social worker.
