We are seeking three experienced and dedicated Adult Social Workers to join our dynamic Care Review Team in Carlisle. This vital role ensures adults receive appropriate and effective care, promoting their wellbeing and independence. We have opportunities for experienced practitioners in two distinct areas:
Two posts will focus on: Reviewing individuals with high-cost care packages. You will work to identify those eligible for Fully Funded Continuing Healthcare (FFCH), progressing cases through the Decision Support Tool (DST) to evidence primary health needs.
One post will focus on: Reviewing individuals receiving direct payments who frequently accrue high account balances. You will investigate and recommend solutions for improved financial management.
About the Role:
- Comprehensive Case Reviews: Conduct thorough reviews of complex cases, ensuring care plans are effective, safe, and appropriate.
- Data Analysis & Evaluation: Critically analyze case information, identifying areas for improvement in care planning and intervention strategies.
- Risk Assessment: Assess risk and make informed recommendations for ongoing support.
- Multi-Agency Collaboration: Work effectively with individuals, families, and other agencies to coordinate support.
- Policy Compliance: Ensure all practice adheres to relevant legislation, policies, and procedures.
- Report Writing: Produce clear, concise reports, and maintain accurate case records.
- Expert Guidance: Offer expert advice and guidance to frontline social work teams.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Significant experience within Adult Social Care, including working with complex cases and financial management.
