Qualifed Adult Social Worker
RIACT TEAM
The RIACT team works closely with our NHS colleagues to achieve the best possible outcomes for individuals and their families. Our primary focus is a Home First approach, facilitating safe and timely hospital discharges and supporting people who experience unplanned situations at home.
This is a fast-paced role where you'll use your skills to make timely decisions with the full support of an experienced Team Manager, Senior Practitioner, and your fellow team members. You will undertake a range of assessments, including:
Functional assessments
Care Act assessments
Safeguarding and Mental Capacity Act (MCA) assessments
Continuing Health Care assessments

What We're Looking For
We need skilled Social Workers who are confident in gathering information and making appropriate, timely decisions. You should have demonstrable assessment and care management skills, along with a drive and passion to make a difference. As a Social Worker in our team, you will:
Build collaborative relationships with key partners, including acute and primary health care, as well as the voluntary sector.
Conduct strength-based assessments and interventions focused on the whole person.
Support adults with care needs and their carers to achieve their desired outcomes and well-being.
Develop personalised care and support plans in line with national legislation, including the Care Act and the Mental Health Act.
Qualifications and Requirements
Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW).
Current registration with Social Work England.
Proven post-qualification experience in a relevant social care setting.
Knowledge of relevant legislation, including the Care Act, Mental Health Act, and Mental Capacity Act.
A valid UK driving license and access to your own transport for business use.
The successful candidate will be required to undergo an enhanced Disclosure and Barring Service (DBS) check.
