Team: Service Manager - Hospital and Front Door Services
Role : Full Time
Gateshead Council is seeking an experienced and passionate Adult Social Worker to lead our Hospital and Front Door Services team. This is a fantastic opportunity to make a real difference in the lives of vulnerable adults in our community, ensuring they receive the right support at the right time.
About the Role and Responsibilities:
- Strategic Leadership: Provide leadership and motivation to a team of social workers, ensuring high-quality service delivery and staff well-being.
- Strategic Development: Work closely with senior management to develop and implement service strategies, policies, and procedures aligned with council priorities.
- Effective Service Delivery: Oversee the day-to-day operations of the team, ensuring that assessments and support are conducted efficiently and effectively, meeting the needs of service users.
- Championing Person-Centered Practice: Promote person-centered practice, ensuring services empower individuals to achieve their goals and live fulfilling lives.
- Building Strong Partnerships: Establish and maintain positive relationships with key stakeholders, including hospitals, other agencies, voluntary organizations, and community groups, to deliver holistic support to service users.
- Resource Management: Manage budgets, resources, and staffing to ensure efficient service delivery and financial sustainability.
- Continuous Improvement: Lead and participate in quality improvement initiatives, ensuring services meet the highest standards and are continually evolving to meet changing needs.
- Hospital Discharge and Front Door Support: Provide direct support to the team by ensuring smooth transitions for individuals being discharged from hospital and by providing accurate assessments and information for those accessing social care services for the first time.
- Collaborative Practice: Work collaboratively with the team, individuals, families, and other professionals, including health services, housing providers, and the police, to deliver timely and effective support.
- Expertise and Guidance: Offer information, advice, and support to team members on safeguarding issues and best practice.
- Accurate Record Keeping: Maintain accurate records and documentation, ensuring adherence to relevant legislation and guidelines.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Proven experience in a senior management role within Adult Social Care, ideally with experience in hospital discharge or front door services.
- Access to a vehicle for business purposes.
- Experience in leading and motivating teams.
- Strong strategic thinking and planning skills.
- Excellent communication, interpersonal, and negotiation skills.
- Proven ability to manage budgets and resources effectively.
- Commitment to person-centered practice and a commitment to improving the lives of vulnerable adults.
- Demonstrated ability to use Microsoft 365 suite apps.
- Resilience to manage the demands of the role.
- A solution-focused approach to problem-solving.
- Flexibility in adapting to the changing needs of the role.
