The Role
Job Title: Social Worker - Cognitive and Physical Disability Team
Employer: Leicestershire County Council
Rate of Pay: £34 per hour
As a Social Worker in this specialist team, you will manage a varied caseload, undertaking comprehensive assessments and developing person-centred care and support plans. Your primary responsibilities will include:
Conducting Care Act Assessments for adults with a range of needs, including physical disabilities, learning disabilities, and autism, to identify their needs and personal outcomes.
Developing, implementing, and reviewing personalised care and support plans that promote independence, choice, and control, including arranging direct payments, personal budgets, and community-based support.
Undertaking Mental Capacity Assessments and initiating Deprivation of Liberty Safeguards (DoLS) applications when required, ensuring individuals' rights are protected.
Responding to and managing safeguarding concerns for adults at risk in line with the Care Act 2014 and local safeguarding adults' policies.
Working collaboratively with a wide range of multi-agency partners, including health services, housing providers, and the voluntary sector, to ensure integrated support.
Advocating for individuals and ensuring their voices, wishes, and feelings are heard and respected in all decision-making processes.
Maintaining accurate, analytical, and up-to-date electronic records in compliance with statutory requirements and Leicestershire County Council policies.
About You
The council is looking for a highly motivated and empathetic Social Worker with:
A recognised Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW) with current Social Work England registration.
A full UK driving licence and access to a car for business use (or ability to travel efficiently around the county).
