Job Title: Social Worker - Community Team

Nottingham City Council is seeking a highly motivated Social Worker to join our Adult Community Team. This is a vital role where you will support adults with diverse needs to maintain their independence, enhance their well-being, and connect with their local community.
About the Role:
As a Social Worker within an Adult Community Team, you will be responsible for assessing the needs of adults, including older people, individuals with physical disabilities, learning disabilities, mental health needs, and their carers. You will develop person-centred support plans, advocate for individuals, and work collaboratively with health partners, voluntary organisations, and other agencies to ensure integrated care and support.
Your responsibilities will include:
- Conducting comprehensive strengths-based assessments for adults with care and support needs.
- Developing and reviewing person-centred care plans, focusing on maximising independence and promoting well-being.
- Safeguarding vulnerable adults from abuse and neglect, adhering to the principles of the Care Act 2014.
- Providing information, advice, and guidance to individuals and their carers.
- Working collaboratively within a multi-disciplinary team, including occupational therapists, physiotherapists, and health professionals.
- Managing a diverse caseload, prioritising effectively, and meeting statutory requirements.
- Promoting community inclusion and signposting to relevant local services.
About You:
We are looking for an individual who is committed to adult social care, with a strong understanding of relevant legislation and a compassionate, person-centred approach.
You will need:
- A recognised Social Work qualification.
- Current Social Work England registration.
- Experience of working within adult social care, ideally CHC, hospital or Long Term Case experience.
- A good understanding of the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.
- Excellent assessment, communication (verbal and written), and analytical skills.
- The ability to work autonomously and as part of a supportive team.
- A commitment to continuous professional development and reflective practice.
- A person-centred approach, valuing choice, control, and dignity.
