The Role:
As a Social Worker within our Adult Assessment team, you will be responsible for undertaking comprehensive assessments of individuals with a range of needs, including:
- Physical disabilities
- Learning disabilities
- Mental health challenges
- Age-related needs
You will work collaboratively with individuals, their families, carers, and other professionals to identify needs, develop support plans, and ensure the delivery of high-quality, person-centred services.
Key Responsibilities:
- Conducting holistic and timely assessments in accordance with relevant legislation, policies, and procedures.
- Developing and implementing outcome-focused support plans in partnership with service users.
- Working effectively with a multi-agency approach, liaising with health professionals, housing providers, and other relevant stakeholders.
- Safeguarding vulnerable adults and adhering to safeguarding policies and procedures.
- Maintaining accurate and up-to-date records.
- Contributing to team meetings, case discussions, and service development.
- Participating in ongoing professional development and training.
- If you have an MHO qualification or are a Care Officer that would be an additional plus
- Knowledge of remote/rural social work would be a bonus.
- There is no hybrid working
Qualifications and Skills:
- A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW).
- Registration with the Scottish Social Services Council (SSSC) as a Social Worker.
- A full driving licence and access to a vehicle.
