Job Title: Part-Time Social Worker - Children with Disabilities Team

Our CWD Team works in partnership with families to provide holistic assessments and coordinated support.
The Role
As a Part-Time Social Worker in the Children with Disabilities Team, you will manage a varied caseload of children and young people with complex disabilities and their families. Your key responsibilities will include:
Undertaking comprehensive assessments of the needs of disabled children, young people, and their families, ensuring their voices are central to the process.
Developing and implementing personalised care plans that promote independence, well-being, and inclusion, including arrangements for short breaks, direct payments, specialist equipment, and community-based support.
Working collaboratively with multi-agency partners including health services (e.g., therapists, paediatricians), education (e.g., SEND teams, special schools), and the voluntary sector, to ensure integrated support.
Leading on transition planning for young people as they move into adulthood (often from age 14 onwards), liaising closely with adult social care services.
Safeguarding disabled children and young people, identifying and responding to any concerns in line with local procedures.
Advocating for the rights and choices of disabled children and their families, ensuring access to appropriate services and opportunities.
Maintaining accurate, analytical, and up-to-date electronic case records in compliance with statutory requirements and Dudley Council policies.
About You
We are looking for a highly empathetic and skilled Social Worker with:
A recognised Social Work qualification.
Current registration with Social Work England.
Minimum 3 years post qualifying experience of working with children, young people, and families, with direct experience in a Children with Disabilities team or working with disabled individuals being highly desirable.
A strong understanding of relevant legislation, including the Children Act 1989, the Care Act 2014 (as it relates to young adults in transition), and the SEND Code of Practice.
Excellent assessment, planning, communication, and interpersonal skills, with the ability to build rapport and work sensitively with diverse families.
The ability to manage a caseload effectively, prioritise tasks, and work independently.
A commitment to strengths-based, child-centred, and inclusive practice.
A full UK driving licence and access to a car for business use (or ability to travel efficiently around the borough).
Availability for part-time hours, which can be discussed to find a suitable working pattern.
