We are seeking a highly motivated and skilled Adult Social Worker to join our dedicated Hospital Social Work Team in Newcastle upon Tyne. This is an exciting opportunity to play a crucial role in supporting individuals and their families during critical periods of transition from hospital to home or other appropriate settings.
About the Role
Working as part of a multi-disciplinary team, you will be responsible for providing high-quality social work interventions to adults admitted to hospital, focusing on their discharge planning and ongoing care needs. Your key responsibilities will include:
- Undertaking comprehensive social care assessments under the Care Act 2014, identifying individual needs and strengths.
- Developing and implementing person-centred care plans, ensuring seamless transitions from hospital.
- Working collaboratively with patients, their families, carers, and a range of health professionals (doctors, nurses, therapists) to facilitate safe and timely discharges.
- Providing information, advice, and advocacy to empower individuals to make informed decisions about their care.
- Identifying and mitigating risks, safeguarding vulnerable adults, and responding appropriately to safeguarding concerns.
- Navigating complex situations, including mental capacity assessments and best interest decisions.
- Brokering and reviewing packages of care, utilising community resources effectively.
- Maintaining accurate and up-to-date records in line with professional and organisational standards.
About You
To be successful in this role, you will possess:
- A recognised Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW).
- Current registration with Social Work England.
- Significant post-qualifying experience as an Adult Social Worker, ideally within a hospital or fast-paced community setting.
- A strong understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults.
- Excellent assessment, analytical, and problem-solving skills.
- Exceptional communication and interpersonal skills, with the ability to build rapport quickly and work effectively with diverse individuals and teams.
- The ability to work autonomously and as part of a team, managing a diverse caseload effectively under pressure.
- A commitment to continuous professional development and reflective practice.
