The Role:
As a Social Worker in the LD Team, you will be responsible for a varied caseload, undertaking comprehensive assessments, developing person-centred care plans, and ensuring the delivery of appropriate support to adults with learning disabilities and their families/carers. This will involve:
- Undertaking Needs Assessments and Care Act Assessments.
- Developing and reviewing support plans in collaboration with individuals, families, and other professionals.
- Safeguarding vulnerable adults in line with Rotherham's policies and procedures.
- Working collaboratively with multi-agency partners including health, housing, education, and voluntary sector organisations.
- Advocating for individuals and ensuring their voices are heard in decision-making processes.
- Promoting independence, choice, and control for individuals with learning disabilities.
- Maintaining accurate and up-to-date records.
About You:
We are looking for a highly motivated and experienced Social Worker with:
- A recognised Social Work qualification.
- Current registration with Social Work England.
- Minimum 3 years of post-qualifying experience in working with adults with learning disabilities.
- A strong understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults.
- Excellent assessment, analytical, and communication skills.
- The ability to work autonomously and as part of a team.
- A commitment to promoting the rights and well-being of individuals with learning disabilities.
- A full UK driving licence and access to a car for business use (or ability to travel efficiently around the borough).
