The Opportunity:
Are you a skilled and experienced social worker with a passion for adult social care? Herefordshire Council is seeking a highly motivated Adult Social Worker to join our Care Act Assessment Team. This is a pivotal role where you will be at the forefront of implementing the principles of the Care Act 2014, working directly with adults and their carers to assess needs and promote positive outcomes.
Key Responsibilities:
As an Adult Social Worker in the Care Act Assessment Team, you will:
Conduct comprehensive, person-centred assessments under the Care Act 2014 for adults with a range of needs, including physical disabilities, learning disabilities, mental health conditions, and older people's services.
Carry out robust eligibility assessments and develop creative, proportionate, and effective care and support plans.
Undertake carer's assessments to determine the support needs of family members and friends providing care.
Work collaboratively with a wide range of professionals, including health services, housing providers, and community organisations, to ensure a seamless and integrated approach to care.
Promote independence, choice, and control for individuals, using a strengths-based approach to support them in achieving their personal goals.
Maintain accurate, timely, and comprehensive case records on the electronic case management system.
Participate in regular professional supervision and team meetings, contributing to a culture of continuous learning.
What We Are Looking For:
A qualified Social Worker with current registration with Social Work England.
Proven post-qualifying experience in adult social care, with demonstrable experience of conducting Care Act assessments.
Strong knowledge and understanding of relevant adult social care legislation and statutory guidance, particularly the Care Act 2014.
Excellent assessment, analytical, and decision-making skills, with the ability to manage complex cases under pressure.
Exceptional communication and interpersonal skills, capable of building rapport and engaging effectively with adults, their families, and professionals from diverse backgrounds.
The ability to prioritise and manage a demanding caseload efficiently.
A valid UK driving license and access to a vehicle for work purposes across the county.
