The Role
Job Title: Social Worker - Mental Health and Reablement Team
Employer: Leicestershire County Council
Rate of Pay: £34 per hour
As a Social Worker in this specialist team, you will manage a varied caseload, providing time-limited, goal-oriented support. Your primary responsibilities will include:
Conducting comprehensive Care Act Assessments for adults with mental health needs to identify their strengths, goals, and support requirements.
Developing and implementing reablement plans and short-term interventions that are focused on recovery and promoting independence.
Working collaboratively as part of a multi-disciplinary team, alongside health professionals such as mental health nurses, psychiatrists, and occupational therapists.
Undertaking Mental Capacity Assessments and initiating Deprivation of Liberty Safeguards (DoLS) applications when required, ensuring individuals' rights are protected.
Responding to and managing safeguarding concerns for adults at risk, in line with the Care Act 2014 and local safeguarding policies.
Advocating for individuals, ensuring their voices and wishes are heard and respected in all decision-making processes.
Maintaining accurate, analytical, and up-to-date electronic records in compliance with statutory requirements and Leicestershire County Council policies.
About You
The council is looking for a highly motivated and empathetic Social Worker with:
A recognised Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW) with current registration with Social Work England.
A full UK driving licence and access to a car for business use (or ability to travel efficiently around the county).
