🌟 The Opportunity & Your Role
As a Community Support Worker, you will be on the front lines, providing vital practical and emotional support to individuals facing challenges related to mental health and substance misuse (including drugs and alcohol).
Key Responsibilities:
Providing tailored one-on-one community-based support and floating support.
Assisting service users with daily living skills, accessing resources, and managing appointments.
Encouraging engagement in positive community activities and social inclusion.
Working collaboratively with external agencies and healthcare professionals.
Maintaining accurate records and contributing to the development of personalised support plans.
