Summary
We are seeking a dedicated and experienced Community Adults Social Worker to join our team in Warrington, providing comprehensive assessment, support planning, and care management services to adults in the community. The successful candidate will be committed to promoting independence, choice, and control, adhering strictly to the principles of the Care Act 2014.
Key Responsibilities
Assessment and Review: Conduct person-centred Care Act Assessments for adults with various needs, including physical disabilities, learning disabilities, mental health issues, and older people.
Support Planning: Develop, implement, and review complex support plans that meet assessed needs, promote well-being, and ensure value for money, focusing on community-based solutions and promoting independence.
Safeguarding: Act as a lead professional in safeguarding matters, managing complex Safeguarding Adults inquiries, and participating in strategy meetings and case conferences.
Risk Management: Undertake comprehensive risk assessments and develop robust management plans to ensure the safety and well-being of vulnerable adults in the community.
Mental Capacity: Conduct and document assessments of Mental Capacity in line with the Mental Capacity Act 2005 (MCA) and Best Interests principles.
Multi-Agency Working: Collaborate effectively with partners in Health (e.g., GPs, District Nurses), Housing, the Voluntary Sector, and other internal council services to ensure integrated support delivery.
Documentation: Maintain accurate, timely, and compliant records on the electronic case management system, ensuring all work meets statutory and local standards.
Essential Requirements
Qualification: Must be a Qualified Social Worker (QSW).
Registration: Current registration with Social Work England (SWE).
Experience: Proven, recent experience working within a statutory Adults Social Care setting (Community, Locality, or Integrated Teams).
Knowledge: Expert working knowledge of the Care Act 2014, the Mental Capacity Act 2005, and local safeguarding procedures.
Skills: Excellent communication, assessment, risk management, and organizational skills.
