Stockton-on-Tees Borough Council is seeking a dedicated and compassionate Qualified Social Worker to empower adults with learning disabilities. Join our supportive team and contribute to person-centred care that promotes independence and well-being.
Competitive Pay: £20 - £26 per hour
Your Role & Responsibilities will include:
Conducting comprehensive assessments to identify individual needs and preferences.
Developing, implementing, and reviewing personalized care and support plans.
Managing a complex caseload, including safeguarding referrals and risk assessments.
Collaborating with multidisciplinary teams, families, and external agencies.
Advocating for the rights and interests of individuals within the community.
Maintaining accurate and up-to-date records in compliance with standards.
Participating in statutory duties (e.g., Care Act assessments, Mental Capacity Act).
Key Requirements:
You must be a Qualified Social Worker.
Social Work England registration is essential.
Updated DBS is required.
