Are you a dedicated and compassionate Social Worker looking to make a genuine difference in the community? South Tyneside Council is seeking a proactive professional to join our Adult Social Care team. This role is focused on empowering individuals, promoting independence, and delivering high-quality, person-centred support.
The Role
As a Social Worker in our Adult Integrated Care Services, you will manage a varied caseload, including complex cases where personal liberty or safety may be at risk. You will be at the forefront of implementing strengths-based approaches, ensuring that support is focused on outcomes rather than being purely service-driven.
Key Responsibilities
Assessment & Planning: Conduct person-centred assessments and work collaboratively with individuals and carers to develop support plans that meet eligibility needs within resource allocations.
Independence & Wellbeing: Promote independence through personalisation, including individual budgets, direct payments, and self-directed support.
Safeguarding: Actively embed the 'Making Safeguarding Personal' ethos, undertaking investigations and managing risks positively to protect vulnerable adults.
Collaboration: Work effectively with multi-agency partners and other professionals to ensure holistic care.
Professionalism: Maintain high-quality data records, provide written reports for court proceedings when required, and participate in regular reflective supervision.
Community Connection: Develop deep links within the local community to help individuals access universal and non-care services.
What We Are Looking For
Qualified Social Worker: You must be registered with Social Work England (formerly HCPC) and maintain an up-to-date record of professional development.
Experienced Practitioner: Ability to manage complex caseloads and provide support/guidance to less experienced staff.
Outcome-Focused: A commitment to strengths-based practice and the ability to adapt to changes in legislation and Council policy.
Strong Communicator: Excellent report-writing skills and the ability to represent the service confidently in professional and legal settings.
