Position description / Job description- Conduct comprehensive needs assessments for adults requiring social work support.
- Develop, implement, and monitor care plans tailored to individual needs and circumstances.

- Provide direct support and advocacy to clients, ensuring their rights and well-being are protected.
- Maintain accurate and up-to-date case records, adhering to professional standards and regulatory requirements.
- Participate in case reviews and contribute to the ongoing development of service delivery.
- Network and liaise with relevant agencies and organizations to facilitate holistic care.
- Contribute to the overall improvement of social work practices and service quality.
We're looking for you, if:
- You hold a relevant social work qualification (e.g., MSW, BA in Social Work).
- You possess a minimum of 2 years of post-qualification experience in adult social work.
- You have experience working with adults with complex needs.
- You demonstrate a strong understanding of relevant legislation and policies.
- You possess excellent communication, interpersonal, and problem-solving skills.
- You are a highly motivated and compassionate individual with a commitment to client-centered care.
- Experience as an AMHP is highly desirable.
