The Role
Middlesbrough Council is seeking dedicated and experienced Adult Social Workers to join our established East and West Locality Teams. This is a fantastic opportunity to work within a local authority committed to delivering high-quality, person-centred care to the residents of Middlesbrough.
As part of the Locality Team, you will work with a diverse range of adults, providing support to help them maintain independence and improve their quality of life.
Key Responsibilities
Holistic Assessments: Conduct comprehensive Care Act assessments to identify the needs and outcomes of adults within the locality.
Support Planning: Develop, implement, and review support plans that empower individuals while managing risks effectively.
Safeguarding: Manage safeguarding adults' enquiries, ensuring that vulnerable individuals are protected from harm and their voices are heard.
Collaboration: Work closely with multidisciplinary partners, including health professionals, housing, and voluntary services, to ensure a seamless approach to care.
Case Management: Maintain an active caseload, ensuring all documentation is accurate, up-to-date, and compliant with statutory requirements.
What We Are Looking For
Qualified Professional: A recognized Social Work degree (BA/BSc/MA/MSc) and a current registration with Social Work England.
Experience: Previous experience working within Adult Social Care, preferably within a locality or community-based setting.
Knowledge: A strong understanding of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.
Communication: Excellent interpersonal skills with the ability to build rapport with service users, families, and professionals.
Resilience: The ability to manage a busy caseload and adapt to the evolving needs of the community.
