The Role
Are you an efficient, autonomous Social Worker with a passion for Self-Directed Support (SDS)?
East Lothian Council is looking for a dedicated practitioner to join their Adult Social Work team for a high-intensity, 12-week project. Based out of the historic John Muir House in Haddington, you will manage a diverse caseload, ranging from young adults (16+) to the elderly.
This is a fast-paced role focused on clearing assessment backlogs, designing community interventions, and securing long-term care placements.
Key Responsibilities
SDS Assessments: Completing robust Self-Directed Support assessments for a broad adult population.
Care Planning: Assessing for longer-term care needs, frailty, and residential placements.
Community Intervention: Providing immediate support and crisis intervention where necessary.
Throughput: Maintaining a high volume of assessments and reviews to meet the project's 12-week goals.
The Requirements
SDS Expertise: Proven experience in Self-Directed Support and community care planning.
Mobility: A full UK Driving Licence and access to a car are essential due to the rural nature of the East Lothian patch.
Tech Savvy: Strong IT literacy is required (Experience with MOSAIC is a significant advantage).
Compliance: SSSC registration and ideally Council Officer Training (ASP).
Office Presence: While one day of WFH is possible, the team is looking for a practitioner who prefers an office-based environment to collaborate with colleagues.
