As a Social Worker in Northumberland's Adults Community Team, you will play a critical role in delivering high-quality, person-centred support to adults with care and support needs. You will be responsible for carrying out Care Act assessments, developing support plans, and working in partnership with individuals, families, and multi-agency professionals to promote independence and wellbeing in the community.
Key Responsibilities:
Undertake strengths-based assessments in line with the Care Act 2014, focusing on individual outcomes and wellbeing.
Co-produce person-centred care and support plans with adults and carers, promoting independence and choice.
Respond to safeguarding concerns, ensuring statutory duties are fulfilled under the Care Act and Safeguarding Adults framework.
Work collaboratively with health professionals, housing, voluntary services, and other agencies to coordinate care and support.
Maintain accurate, up-to-date case records using the council's electronic case management system.
Support hospital discharges and prevent admissions by promoting reablement and preventative services.
Apply relevant legislation (e.g., Mental Capacity Act, Deprivation of Liberty Safeguards) in day-to-day practice.
Participate in regular supervision, team meetings, and contribute to the continuous improvement of the team's service.
Person Specification:
Qualified Social Worker with SWE (Social Work England) registration.
Experience of working within adult social care, particularly in a community setting.
Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures.
Effective communication, assessment, and decision-making skills.
Ability to manage a varied and sometimes complex caseload.
Comfortable working independently and as part of a multi-disciplinary team.
Experience using case management systems is desirable (e.g., Liquidlogic).
