We are seeking a highly motivated and experienced Social Worker to join our dedicated Cared For Children Team in Middlesbrough. You will work directly with children and young people who are looked after by the local authority, focusing on those with a plan to live outside of the family home. You will be responsible for supporting these young people and their families to achieve positive outcomes and prepare for independent living.
Key Responsibilities:
* Conduct regular reviews and assessments of children in care, focusing on their needs and progress towards their care plans.
* Develop and implement person-centred care plans that reflect the individual needs and aspirations of each child. This includes working with the child, their family (where appropriate), and other professionals to identify goals and strategies to support their transition to independence.
* Work closely with foster carers and adoptive families, providing guidance, support, and regular supervision. This involves building strong working relationships based on trust and mutual respect.
* Maintain regular contact with children and young people in care, providing support and guidance.
* Prepare and present comprehensive reports for case reviews and court hearings. This requires excellent writing and presentation skills and the ability to present complex information clearly and concisely.
* Liaise effectively with a wide range of professionals and agencies, including education, health, and youth justice services. This requires excellent communication and collaboration skills.
* Ensure all casework is compliant with relevant legislation and regulations. This involves maintaining accurate records and adhering to all policies and procedures.
* Participate in regular supervision and training opportunities. This will provide ongoing professional development and support.
* Contribute to the ongoing development and improvement of the team's services.
Essential Criteria:
* A recognised Social Work qualification.
* Current registration with the Health and Care Professions Council.
* Significant post-qualifying experience in children's social work, with demonstrable experience working with looked after children.
* Excellent communication, assessment, and care planning skills.
* Ability to work independently and as part of a multi-disciplinary team.
* Full UK driving licence and access to a vehicle.
Desirable Criteria:
* Experience working with children with specific needs (e.g., emotional and behavioural difficulties, disabilities).
* Knowledge of relevant legislation and policy frameworks.
* Experience with case management software.
