Northumberland County Council is seeking a highly motivated and skilled Social Worker / Practitioner to join our established and supportive Disabled Children's Team. This is your chance to make a real, lasting difference in a county we believe is the best place to live and work.
About the Role
Working within our multi-agency team, you will be responsible for providing comprehensive statutory social work support and assessment for disabled children and young people aged 0-25 and their families.
Your key responsibilities will include:
Conducting high-quality Children and Family Assessments and Early Help Assessments to determine the needs of the child and family.
Developing, implementing, and reviewing robust Care Plans that promote the child's welfare and encourage their inclusion within the community.
Supporting young people and their families through key transitions, particularly into adulthood and adult services, from the age of 14.
Undertaking direct work with children and families, building positive, trusting relationships.
Managing a diverse and complex caseload, including Child in Need, Child Protection, and Looked After Children cases, where appropriate.
Working in partnership with health, education, and voluntary sector professionals to deliver a seamless service.
What We're Looking For
A relevant professional qualification in Social Work (for Social Worker role) and current registration with Social Work England.
A strong understanding of relevant legislation, policy, and practice relating to disabled children and their families (e.g., Children Act, Care Act, SEND Code of Practice).
Experience working with children, young people, and adults with a range of complex needs, including physical, learning, and dual sensory disabilities (desirable).
Excellent communication, assessment, and report-writing skills.
